RESUMO

STATEMENT OF PROBLEM: The stability of mandibular removable partial dentures with bilateral distal extensions may be improved with the controlled tissue support achieved by using the altered cast impression technique, although this process is time-consuming and technique-sensitive. PURPOSE: The purpose of this pilot clinical study was to compare casts generated from a conventional definitive impression with casts generated from an altered cast impression using a 3-dimensional (3D) analysis software program. MATERIAL AND METHODS: Three partially edentulous participants with mandibular Kennedy Class I were enrolled, and impressions were made with the 2 techniques and poured in stone. The casts were scanned, aligned, and superimposed by using a 3D analysis software program. Surface deviations were measured to evaluate the differences in displacement induced by the impression on the tissue surface. Five observations were made in 4 different areas on each partially edentulous side. Means from these observations were generated, and the Wilcoxon and Mann Whitney tests were performed for all data to assess the differences between the right and left sides in the same participant and among the 3 participants (α=.05). RESULTS: The casts made from the altered cast impression had an overall mean ±standard deviation displacement of -0.05 ±1.25 mm on the right and left sides of the mandibular buccal shelf area. Moreover, the greatest overall difference of about 0.45 ±0.41 mm occurred on the lingual slope of the residual ridge, and the differences in the other areas were 0.10 ±0.99 mm (crest of the residual ridge) and 0.16 ±0.66 mm (buccal slope of the residual ridge). The overall differences varied statistically between significance and nonsignificance for the same participant and among the 3 participants. CONCLUSIONS: A digital comparative analysis of the conventional and altered cast methods of recording the bilateral distal extension areas in partially edentulous participants showed that the altered cast method exhibited more displacement on the buccal vestibule or buccal shelf area compared with other examined areas. The differences between the 2 impression methods in the displacement values among the examined areas were minimal and in close proximity, and such differences may lack clinical significance.

INTRODUCTION: Root canal sealing materials play a crucial role in an endodontic procedure by forming a bond between the dentinal walls and the gutta-percha. The current study aims to analyse the dentinal tubule penetration and adhesive pattern, including the push-out bond strength of six commercially available root canal sealers. METHODOLOGY: Eighty-four mandibular first premolars were split into seven groups (and n = 12), Group 1: Dia-Root, Group 2: One-Fil, Group 3: BioRoot RCS, Group 4: AH Plus, Group 5: CeraSeal, Group 6: iRoot SP, Group 7: GP without sealer (control). Two groups were made, one for dentinal tubule penetration and the other for push-out bond strength; the total sample size was one hundred sixty-eight. Root canal treatment was performed using a method called the crown down technique, and for obturation, the single cone technique was used. A confocal laser scanning microscope (Leica, Microsystem Heidel GmbH, Version 2.00 build 0585, Germany) was used to evaluate dentinal tubule penetration, and Universal Testing Machine was utilised to measure the push-out bond strength (Shimadzu, Japan) using a plunger size of 0.4 mm and speed of 1mm/min. Finally, the adhesive pattern of the sealers was analysed by HIROX digital microscope (KH-7700). Statistical analysis was carried out by a one-way Anova test, Dunnet's T3 test, and Chi-square test. RESULTS: Highest dentinal tubule penetration was noticed with One-Fil (p<0.05), followed by iRoot SP, CeraSeal, AH Plus, Dia-Root also, the most negligible value was recorded for BioRoot RCS. Meanwhile, BioRoot RCS (p<0.05) demonstrated the greater value of mean push-out bond strength, followed by One-fil, iRoot SP, CeraSeal, AH Plus and Dia-Root. Regarding adhesive pattern, most of the samples were classified as type 3 and type 4 which implies greater sealing ability and better adherence to the dentinal wall. However, BioRoot RCS revealed the most type 4 (p<0.05), followed by AH Plus, One-Fil, CeraSeal and Dia-Root. CONCLUSION: The highest dentinal tubule penetration was shown by One-Fil compared to other groups. Meanwhile, BioRoot RCS had greater push-out bond strength and more adhesive pattern than other tested materials.

INTRODUCTION: Cleft lip and palate (CLP) are the most common congenital craniofacial deformities that can cause a variety of dental abnormalities in children. The purpose of this study was to predict the maxillary arch growth and to develop a neural network logistic regression model for both UCLP and non-UCLP individuals. METHODS: This study utilizes a novel method incorporating many approaches, such as the bootstrap method, a multi-layer feed-forward neural network, and ordinal logistic regression. A dataset was created based on the following factors: socio-demographic characteristics such as age and gender, as well as cleft type and category of malocclusion associated with the cleft. Training data were used to create a model, whereas testing data were used to validate it. The study is separated into two phases: phase one involves the use of a multilayer neural network and phase two involves the use of an ordinal logistic regression model to analyze the underlying association between cleft and the factors chosen. RESULTS: The findings of the hybrid technique using ordinal logistic regression are discussed, where category acts as both a dependent variable and as the study's output. The ordinal logistic regression was used to classify the dependent variables into three categories. The suggested technique performs exceptionally well, as evidenced by a Predicted Mean Square Error (PMSE) of 2.03%. CONCLUSION: The outcome of the study suggests that there is a strong association between gender, age, and cleft. The difference in width and length of the maxillary arch in UCLP is mainly related to the severity of the cleft and facial growth pattern.

BACKGROUND: The study aimed to evaluate 1) the amount of color variations presents within clinical images of maxillofacial prosthetic silicone specimens when photographed under different clinically relevant ambient lighting conditions, and 2) whether white balance calibration (WBC) methods were able to mitigate variations in ambient lighting. METHODS: 432 measurements were acquired from standardized images of the pigmented prosthetic silicone specimens within different ambient lighting conditions (i.e., 2 windowed and 2 windowless clinics) at noon with no light modifying apparatus. The specimens were photographed once without any white balance calibration (raw), then independently alongside an 18% neutral gray card and Macbeth color chart for calibration in a post-processing (PPWBC) software, and once after camera calibration (CWBC) using a gray card. The LAB color values were extracted from the images and color variations (ΔE) were calculated after referring to the corresponding spectrophotometric values as control. RESULTS: Images in windowless and windowed clinics exhibited highly significant differences (p < 0.001) with spectrophotometer (control). CWBC demonstrated no significant differences (p > 0.05) in LAB values across windowed clinics. PPWBC using Macbeth color chart produced no significant differences for a* values (p > 0.05) across all clinics while PPWBC by gray card showed no significant differences (p > 0.05) in LAB values when only similar clinics (either windowed or windowless) were compared. CONCLUSION: Significant color variations were present for maxillofacial prosthetic specimens owing to natural ambient light. CWBC and PPWBC using color charts were more suitable for color correction across windowed clinics while CWBC and PPWBC using gray cards had better outcomes across windowless setups.

Background Color selection for discolored teeth or for edentulous patients is a subjective process in dentistry. To the best of the authors' knowledge, there is no objective method for tooth color selection for edentulous patients. Several studies have investigated the relationship between tooth color and skin color and the ability to use skin color as a guide to select tooth color in dental rehabilitation cases. The objective of this study was to find and model the relationship between tooth color and skin color in equations to be able to predict tooth color by assessing skin color. Methodology This study was done on 150 participants with equal gender distribution. Digital photography with soft boxes and strobe lights were used to measure tooth and skin color. Digital photography was performed in a dark room with controlled and measured lighting sources and strength. A calibration board was used to create two calibration profiles in software. All photos were calibrated in Adobe Lightroom software. Then, color measurement was done using Adobe Photoshop software in the Commission International de I'Eclarirage (CIELab) system. Results There were positive significant relationships in color spaces L* and a* between tooth color and skin color, and an inverted significant relationship in color space b* between tooth and skin. Three equations with excellent reliability were formulated in this study to predict tooth color by assessing skin color. Conclusions The relationship between tooth color and skin color was modeled in three equations. For edentulous patients, this research might be useful in detecting the proper tooth color and transferring the CIELab color data to dental technicians for further processing. Computerized shade matching and prediction dental software may be designed using these equations.

Gingival enlargement may manifest as a side effect of medications (calcium channel blockers, anticonvulsants, or immunosuppressants) and may be associated with inflammation, malignancy, or genetic inheritance. This condition has a significant impact on a patient's quality of life and affects their oral health status. This case report describes the management of a 68-year-old gentleman who presented with generalized gingival enlargement and chronic periapical abscess originating from tooth 34, which served as an abutment for a fixed partial prosthesis. The patient's medical history revealed that felodipine, an antihypertensive medication, was prescribed to him. A comprehensive treatment plan was developed to improve the patient's quality of life.

Patient dissatisfaction with tooth discoloration as a result of tetracycline therapy is not uncommon. To address patients' aesthetic demands, conservative bleaching treatments were considered before more invasive, irreversible treatments such as dental veneers or crowns. Bleaching is a relatively non-invasive, safe, and cost-effective method of achieving a desirable result. However, due to many limiting variables, including the extended duration of active bleaching, tetracycline-stained teeth are one of the most challenging cases to obtain satisfactory bleaching outcomes. This clinical report presents two cases of management of tetracycline staining of the teeth with varying degrees of severity.

BACKGROUND: Two patients received ocular injuries from rusted metallic projectiles at their industrial workplaces. Said injuries resulted in the loss of their eyes by evisceration surgeries to prevent fatal infections. CASE DESCRIPTION: The first case, a man in his twenties, received a stock conformer immediately after surgery and started prosthetic therapy within 2 months. The second case, a man in his forties, started prosthetic therapy after 10 years. Definitive custom ocular prostheses were fabricated and relined according to conventional protocol. RESULTS: On issue of the prosthesis, there was adequate retention, aesthetics and stability to extra-ocular movements and treatment was considered successful for both cases. However, follow-ups showed noticeable prosthetic eye movements for case 1 which, to some extent mimicked the physiologic movement of its fellow natural eye. Case 1 adjusted to his prosthesis better while case 2 was still adjusting with little to no physiologic movement. CONCLUSION: Prosthetic rehabilitation should be started as early as possible to obtain optimum rehabilitative results.
